Dear parents, last month, we have requested that all our students to submit a medical waiver. Forms were both handed out to students in hard copy form as well as emailed in digital form to parents. Despite repeated reminders, we are still missing over 30 of these forms!
If you have not yet filled out a medical waiver form for your child(ren), please check your emails from November 25th. The Omega Front Office sent out an email to all families who still need to submit such a form. Sometimes, these emails end up in the Spam, so check there, too, please. The email originator was omegainfo@mcsed.ca. Also, your child may have a hard copy in their backpack. 🙂
We appreciate your cooperation in ensuring that the school has a medical waiver on file for your child. Forms can be submitted at the Front Office.
(Side note: medical waivers are valid both for sports and field trips, and only have to be filled out once per school year).
